Open Enrollment is an annual opportunity for retirees to review and make changes to their health benefits for the upcoming calendar year.
During Open Enrollment, you may:
- Change your health plan
- Add/remove dependents
- Enroll dependents on the GAP Plan for Surest members (current Gap coverage will continue)

High Deductible Health Plan
- Premium change
- Annual family deductible $3,400 (IRS requirement)
- Insulin (30-day supply): $35 co-pay, deductible waived
- Teledoc general and mental health visits: $0 co-pay, deductible waived
- Coverage for speech-language pathology treatment for stuttering-members age 26 and under*
- Coverage for fertitlity preservation service for patients diagnosed with breast or ovarian cancer, when treatment may cause infertility.*

- Ambulance Ground Transportation co-pay: $350
- Outpatient Surgery co-pay: $35-$3,000
- Inpatient Hospitalization co-pay: $200-$3,000
- Matenity co-pay: $900-$2,000
- Physical Therapy co-pay: $10-$70
- Preferred Brand Medication co-pay: $60 (30-day supply)
- Non-Preferred Brand Medication co-pay: $90 (30-day supply)
- Specialty Medication co-pay: $150-$300
