Form 1095-C contains information about the health coverage that was offered to you or that you and/or your dependents enrolled in for the 2025 tax year.

Under IRS guidance, these forms are available upon request. You will not automatically receive a copy unless you ask for one.

Members enrolled in Surest and Senior Care Plus members will recieve a 1095-C from those respective organizations.
