Coordinated Entry
Connecting People Experiencing Homelessness to Housing and Services
Coordinated Entry is the system used across the Northern Nevada Continuum of Care (CoC) to connect individuals and families experiencing homelessness with housing and supportive services. The system helps ensure that housing resources are distributed fairly and prioritized for those with the greatest needs.
A key part of Coordinated Entry is the Community Housing Assessment Tool (CHAT). CHAT is a standardized housing assessment used by community agencies to better understand an individual or family’s current housing situation and level of need.
During a CHAT assessment, trained staff ask questions about housing history, safety, health, and other factors that may affect housing stability. The results help determine the urgency of housing needs and connect individuals to appropriate housing resources.
CHAT assessments are entered into the Homeless Management Information System (HMIS), a regional database used by service providers across the Northern Nevada CoC. Based on assessment results, individuals may be placed on a prioritized Community Housing List, sometimes called the “Community Queue,” which is used to match people with available housing programs funded by the U.S. Department of Housing and Urban Development (HUD).
Northern Nevada uses a “No Wrong Door” approach, meaning individuals can complete a CHAT assessment through several partner agencies throughout the community.
How Coordinated Entry Works
Step 1: Meet with a Case Worker or Outreach Worker
Individuals experiencing homelessness meet with a trained staff member at one of several participating community agencies.
Step 2: Complete the CHAT Assessment
The Community Housing Assessment Tool (CHAT) is used to understand an individual or family’s housing needs and determine the urgency of their situation.
Step 3: Placement on the Community Housing List
If eligible, individuals are added to a prioritized Community Housing List. People with greater housing needs are placed higher on the list.
Step 4: Referral to Housing Opportunities
As housing becomes available, individuals may be referred to housing programs based on their priority level, eligibility, and program openings.

Most individuals complete a CHAT assessment at the Nevada Cares Campus, but several community partners can also provide assessments.
