This survey will take about five to 10 minutes to complete. It is completely voluntary and confidential. It will not be linked to you or your case. You are also welcome to skip any questions on the survey that you don’t want to answer.
Youth Feedback Survey - These should only be completed while the child is on-site. Concerns regarding confidentiality and privacy mean that mailing or emailing the survey to a child later is inappropriate. We like to get feedback from the youth who come to the CAC, so we can be sure we are doing our best. Please don’t put your name on it so you can keep your answers private. You can ask a staff member any questions or ask for help anytime. Take your time and look it over. If you want to fill it out, great. If not, that’s OK, too.
