This survey will take about five to 10 minutes to complete. It is completely voluntary and confidential. It will not be linked to you or your case. You are also welcome to skip any questions on the survey that you don’t want to answer.

Mental Health Services Survey - This survey is to help us better understand the mental health resources you are currently using, any potential barriers, and the helpfullness of those therapists/counselors.
