This survey will take about five to 10 minutes to complete. It is completely voluntary and confidential. Ity will not be linked to you or your case. You are also welcome to skip any questions on the survey that you don’t want to answer.

Initial Visit Caregiver Survery - We are asking you for feedback about your experience at the CAC. It is important to us that our center is as welcoming, effective, and supportive as possible, and information about your and child’s experience will help us to know what is working well and what we might improve.
