Caregiver Survey

This survey will take about five to 10 minutes to complete. It is completely voluntary and confidential. It will not be linked to you or your case. You are also welcome to skip any questions on the survey that you don’t want to answer.

Caregiver Follow-Up Survey - Caregiver Follow-Up Surveys should be completed by caregivers 60-90 days after their first visit to the center. We ask all caregivers for feedback about our mental health services. It is very important to us to provide counseling services that feel welcoming, safe, helpful, respectful, and engaging for children and their caregivers. Your experiences and suggestions would be incredibly helpful to our center as we assess what is working well and what we could improve about our counseling services.
